Getting FATAL ERROR DB Error: already exists

We've started getting the following error:
FATAL ERROR
DB Error: already exists
..whenever we try to modify an Outbound Route. There is no more information than that being displayed - no indication "what" db or where. I've restarted and rebooted to no avail. I believe the version is 2.4.0.1. This is kind of urgent, as one of our trunks is down, and I can't add a new one to the route.
