Submit button on queue creation non responsive

i have trixbox CE current release is 2.6.0.7
loaded queues and IVR modules via the admin interface.

1) when i use IVR, complete forms, and add "1" "2" and "3" steps at the bottom , press submit clears the "1" "2" and "3" and submits the IVR,,,, edit the IVR ..the steps are there but no numbers in the boxes

2) when I create a queue ...final submit of the queue does not work ..almosy .like the submit button is deactivated ???

no errors in php logs, httpd logs, asterisk logs related to this ..
I have tried to IE, opera and Firefox ..so it does not seem to be a browser issue.

Has anyone else experienced queue submit button not working ..... any suggestions are appreciated ..i need to get some IVR and queues going ASAP
