Web Server crashes on Extension Add

Hi all
hopefully someone has an idea on this one!
I have just installed 2.4.2 to a ProLiant ML110 G3, all looks good, until we get to adding an extension...
At that point the web server stops responding, if you eventually get back to the status page, web server shows as error.
The hang can happen at either the point when you click the Submit for new SIP device etc, or sometimes when you click submit after entering all the extension details.
The server has been yum -y updated and updated from within tb webadmin.
