Ring no voice mail

I'm having an issue were someone is saying that their wife is calling them from a cell phone and the call just rings without going to voice mail. This is a small system that only takes a few thousand calls a month and this is the only person that has reported this problem. All the calls come in through a PRI there is no SIP trunks. Does anyone have any suggestions on were I should start looking.

I got the same thing with my test server. I am using Trixbox 2.6.1.11 with updates from yum.
I thought it may have been a problem with my trunks, routes or dialing plan but this also happens with internal calls.
I can call any extension, from an internal source or external source, the phone rings a few times then stops. No forwarding to voicemail.
Is there a fix?
Followup:
I fixed this. There were missing directories that affected all voicemail activities. Using a working system as my example, I changed all rights and ownership to the proper settings. Everything seems to be working fine.
